That can depend on whether the baby is born before or after the couple marries.
If before, then usually raised as the husband's stepchild but not his primary heir. If after, then legally he/she is a child of the marriage, and the actual paternity just ignored - particularly if the husband really wanted an heir and has no real hope of fathering a child himself....
